If extended an offer to contract with us, you would be acknowledging that you are an independent contractor, and that as an independent contractor, you would not be eligible for any benefits provided by MicahTek to its employees, including, but not limited to, workers’ compensation benefits. In that connection, MicahTek’s workers’ compensation insurance carrier requests the following:
- If you would be working as a sole proprietor with employees or as an entity with employees, both of which are subject to the provisions of the Workers' Compensation Act, you would be asked to provide MicahTek with a “Certificate of Workers’ Compensation Coverage” naming MicahTek, Inc. as an “additional insured” on your workers’ compensation policy.
- If you would not be working as a sole proprietor or an entity subject to the provisions of the Worker's Compensation Act, you would be asked to provide MicahTek with an “Affidavit of Exempt Status Under the Workers’ Compensation Act and Exempt Status Fact Sheet.”
